Thank you for registering your child to participate in our 2022 Vacation Bible School program at Faith Church. We are committed to providing your child with a fun, safe, and meaningful experience. This means that:

1. ALL volunteers will have processed a CORI Background Check.
2. Only parents or individuals listed below will be allowed to pick up your child. Children must be checked in and checked out each night.

We are doing our best to build an inclusive environment at all of our programs. Please share with us any ideas that may help your child, including but not limited to medical, physical, sensory and/or emotional needs. We will provide a supervised "Sensory Safe" alternative for any child that may not want to participate in science experiments or games that may be too much for them.

I hereby consent that my child (named above) may receive medical treatment under the direction of any licensed physician in the event of an injury or medical emergency. This authority is granted only after reasonable effort has been made to contact me by phone listed on this registration form. While I understand that accidents happen, I will not hold Faith Church and all adult volunteers responsible in the event that my child is injured while in participation at the Vacation Bible School program. We are asking for a suggested donation of $20 per child. This will help to cover program expenses, materials, snacks, and provide scholarships for other children to attend for free.
