A Loaner Device Incidental Damage Subscription for your student's laptop will be offered by DSFH for $25 per student.  THIS IS OPTIONAL.  This will not cover intentional or malicious damage.  It will not cover damage due to negligence or theft as determined by administration.  You may purchase insurance through the online payment system link on the DSFH website  where all other fees can be paid.  The Incidental damage subscription is attached in the laptop information section on this orientation page.  If you wish to purchase, please print out that form, fill it out and return it to the office.  You must also go online and pay for the subscription.  The online payment link is also in the orientation information. *

Please read the following carefully:                                             1)   I understand it is my/my student's responsibility to properly maintain this device, and I am responsible to reimburse DSFH for any damage done to the device while in my/my student's possession.    The replacement cost is approximately $250 if this device is destroyed or lost.                                                                             2)    I understand It is my student's responsibility to be prepared daily and come to school with a charged working device.  Failure to do so may result in disciplinary action for your student.                                                                                               3)    I understand I may purchase an optional incidental damage subscription at a cost of $25 per student that will cover any accidental or incidental damage to the laptop while in my/my student's possession.  This will not cover damage done intentionally or due to negligence as determined by administration.                                                                          4)   I understand that this device is to be returned in a timely manner if recalled or at the conclusion of the school year.  The student will be placed on the debt list for the full cost of the device if they fail to return the device.                                                                                        5)   I understand that this device is to be used by only my student and only for educational purposes.  Anything outside of this is prohibited and may result in disciplinary action being taken against them.                 6)   I understand loaner items may not be changed in any way; this includes marking or modifying the appearance of the device in any way. No unapproved adjustments may be made to the devices.                      7)  As parent/guardian I accept responsibility for supervision of my student’s online activity and use of any loaner device when outside of school hours. I am also responsible for any repair and replacement of the loaner device. *
