Entertainment Form - Farmers' Market on Broadway
Apply to perform at the 19th Annual Farmers' Market on Broadway held every Wednesday from May 25th to September 28th.  We will have two areas at the market programed with entertainment: the main stage and north stage (at the Dousman entrance).

Performance hours are from 4-8pm May through August and 3-7pm in September.  We have increased our payment rate this year!  The area by the beer garden pays $300 and the area near the North entrance of the market pays $200.

***Applications are open until Sunday, February 27th.  Artists will then be informed via email on what date and stage they will be performing on a contract will be sent.  This form is for interest purposes only!

Artist must be individually insured and sign a waiver relieving On Broadway, Inc. of any and all liability before, during and after performance.  Artists must also fill out and complete a W9 form.

OBI provides a 10'x10' tent, tent weights and a small whisper quiet Honda 2000 generator (110v outlet).  Artist to provide sound equipment and any other needs for set up (chairs, tables, staging, etc.).  Needs for additional power please describe below:
