Photo/Video Release Waiver *
I acknowledge and consent to allow CityDance Ensemble, Inc/Productions and its assignees to use: photos and/or videos of my child in publications (such as flyers/brochures), advertisements (such as newspapers or online ads), our company’s website, or on social media sites ( including, but not limited to, Facebook, Instagram, Twitter, Pinterest, and YouTube). I understand that in some cases, my child and/or my name may be used. I hereby release you, your employees, and directors from all claims and demands arising out of or in connection with any use of said photo/video material, including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights. I further understand no financial compensation will be for use of these photos/videos.
