Fifty percent (50%) down payment of the total contract price is required upon signing of the reservation form. All remaining charges to be settled on the Event date. Payment may be made in cash or credit card transactions. Personal checks are accepted for down payments only. Payments could also be made through bank deposits under the name First Pacific Leadership Academy, Inc. Metrobank C/A Number: 7-402-90214-7, RCBC C/A Number 5359001187, and BDO S/A Number 4060092897.Please send the deposit slip to FPLA through fax number 696-10-51 loc. 706 (Finance) or you may send it thru email hello@fpla.com.ph , so we can provide the Official Receipt. Any claim for a withholding tax should be supported by a Certificate of Creditable Tax Withheld at Source Form (BIR Form No. 2307). *
For Social Events Cancellation or postponements made 8 days or less prior to the event bookings shall be charged 50% of the contract price. This policy shall not apply in case of fortuitous events. *
I, in my legal capacity, acknowledge and agree that any use of First Pacific Leadership Academy’s (FPLA) facilities, services, and restaurant  (“Tipulo Modern Filipino Cuisine”) come with inherent risks including, but in no way limited to sickness or disease including, without limitation to COVID-19. I voluntarily accept and assume full responsibility for these risks in using Tipulo's service. I agree that I have full knowledge of the nature and extent of all such risks. FPLA shall not be held liable and perpetually discharged for any symptoms, repercussions, or any harmful effects that are in line with COVID-19 which can have an impact during the Clients/Guests exposure at FPLA. *
