Prior to beginning this application/health form you will want to gather the following information:  
1.  Team Name
2.  Date of trip
3.  Participant's health history including immunizations, allergies, and medications
4.  Health insurance information (or traveler's insurance)
5.  Passport Number and expiration date
6.  For MINORS - A legal guardian must be present to complete a portion of this application
7.  For TEAM LEADERS - you will need to know your team demographics
