Horicon Public Library                                                Storytime to Go Sign-Up Form:  Summer 2023
No time to make storytime at the Library, no problem!  Sign up for a Storytime to Go kit to use at home.  Each kit contains 2 books, music cd, a craft, activity sheets, and a snack.  The themes will vary from week to week. Library materials in the kit will be checked out to the parent/caregiver registering the participant. The kits will be available for pickup beginning on Tuesdays and held through Saturday.

This session of Storytime to Go will run from Tuesdays, June 6, through August 29, 2023.  Feel free to sign up any time during the session.

Please fill out this form for each participant, even if they're in the same household, so we may accurately prepare your materials.

Please enter your library card number with no spaces.  A library card is not needed to place your request for a storytime kit. However, a library card is required to take home library owned materials packaged with the kit which you may use for the duration of their usual checkout period.
