We'd like you to complete this survey every time or anytime you meet or gather together as part of Attitude, Neighbourhood House or Men's Shed. It will take only a minute or two of your time to complete.

Only the first 3 questions are required, but we'd appreciate it if you could supply as much feedback as possible. The remaining questions will provide details and insights to help us improve the support Clunes Neighbourhood House can provide to groups. It will also assist us by painting a picture of activity levels and the impact of community initiatives in our town.
