Fabius-Pompey Draft Reopening Plan Survey
Fabius-Pompey has been given guidance from the Governor, the New York State Education Department and the Department of Health for reopening schools.  The District is considering an in-person plan, a remote plan and a hybrid plan.

The top priority is the health, safety, and welfare of our students, staff, and families.  

Fabius-Pompey's DRAFT School Reopening Plan is now posted online.  The District is asking for additional feedback and questions from our families to best serve the needs of our school students.

Families with multiple children are asked to complete ONE survey per family.  Your input and feedback will be appreciated and will help strengthen our planning.  Thank you for participating.

Fabius-Pompey has created plans for three models of instruction.  It is Fabius-Pompey's intention to return all students for in-person instruction. At the same time, the District will accommodate those children who are not able to attend school in person. In creating the educational plans, the District has followed guidance set forth by the local health department, the CDC and the State Education Department for all grades in September. Based on this information you currently have, will you send your child to school in September? *
Are you having your child(ren) ride the bus in the fall given the guidelines that are currently in place (for example: face coverings worn at all times) *
Wellness and Temperature Check information
As demonstrated in the draft plan, students and staff will need to complete a wellness check (via Google forms) including a temperature check each morning before attending school.  Your willingness to complete a wellness check is essential for the reopening of school.  

A wellness and temperature check consists of:

1. Answering the following with a "Yes",  "No" or "unsure" to the following question. Has your child knowingly been in close or proximate contact in the past 14 days with anyone who has tested positive through a diagnostic test for COVID-19 or who has or had symptoms of COVID-19; tested positive through a diagnostic test for COVID-19 in the past 14 days; has experienced any symptoms of COVID-19, including a temperature of greater than 100.0°F in the past 14 days: and/or has traveled internationally or from a state with widespread community transmission of COVID-19 per the New York State Travel Advisory in the past 14 days?

2. Answering the following questions by checking one or more of the following. Does your child have any of the following symptoms? Fever, Cough, Shortness of breath or difficulty breathing, Fatigue, Muscle or body aches, Headache, New loss of sense of taste or smell, Sore throat, Congestion or runny nose, Nausea or vomiting, Diarrhea, or Feels fine

3. Providing your child's temperature for the day? (This is required information.)  
Based on the wellness and temperature check information above, will you be able to perform a daily wellness and temperature check prior to your child(ren) leaving for school? *
Students and staff will be required to wear face coverings at all times with the exception of meals, snack time and designated mask breaks.  Will you be able to provide an appropriate face mask for your child(ren)?
