This form should be completed if your child is currently in receipt of Free School Meals and you wish to obtain a weekly food parcel. If you have multiple children who attend the academy, please complete one form for a child in the household, but identify all children in the final box.

Please ensure all areas of the form are completed accurately. By completing this form, you are consenting to the academy collecting all information provided.

Once complete a member of your child's Small School will contact you to confirm further arrangements.
