Medford High School has a bus for students who live in North Medford where there is no MBTA bus service.
 
Please find the stop closest to you and check that box.

Complete Section 1 and then choose the bus stop in Section 2 (please complete a separate form for each student taking the bus).

Please see your school’s handbook (on the website) for bus riding rules. Only student bus riders may board the bus.
