STUDENT REGISTRATION FORM - Registration is free! Please submit one registration entry form per student, per category.  For example: If you are a student and are submitting an Art entry and a Logo entry, please fill out TWO registration forms, one for each category.  Note: You will need to have your school and home addresses, phone numbers, teacher name, and parent or guardian name and phone number, in order to submit this form. All answers are required. Please answer questions completely.  **Important**  Attach your signed Release and Waiver of Liability Agreement, linked at the end of this form, and a printed copy of your Registration Form securely to the back of your project.

Please use correct spelling and capitalization on this form, this information is used in printed materials and for awards.
