As a volunteer or tutor, I, the undersigned, agree to • abide by and support the philosophy, policy, and guidelines of the organization • act in the best interests of the organization • maintain confidentiality • abide by safety and risk management requirements • carry out the tasks outlined in the Role Description • attend meetings on the agreed time and day • to work without financial reward • to work without benefits, including employment insurance benefits upon the termination of this agreement or as a result of this service • to participate in required physical activity (i.e. standing, lifting and carrying up books & stationery etc..) with knowledge of the hazards and potential injuries involved, and to accept any and all risks of personal injury and property damage • I, and my assignee's, heirs, guardians, and legal representatives, will not make a claim against or sue ‘Hope Avenue’ or its Board members, employees, partners or volunteers for injury or damage resulting from the negligence, whether active or passive, or other acts, however caused, by any of its officers, employees, partners or volunteers of ‘Hope Avenue’ as a result of my volunteering • I, hereby release and discharge ‘Hope Avenue’ and its officers, employees, agents and contractors from all actions, claims, or demands that I, my heirs, guardian and legal representatives now have, or may have in the future, for injury or damage resulting from my participation in the projects • Hope Avenue is not responsible to provide ride to the assignment location • Volunteers are responsible to reach location on time • to be photographed & video recorded during volunteer work and allow usage in any media • maintain a record of my volunteer hours and ensure that information is given/posted • raise any matters of concern with project leader or leadership team.
