Please fill out this form if you would like to report an equity issue. It can be something that happened to you or that you witnessed. Issues about evidence, or arguments are NOT equity issues and should not be reported on this form.

This form will allow you to remain Anonymous, but if you would like follow up on your report you will need to disclose contact information. Please include as many details as you are able and comfortable with sharing. These forms are for reporting ALL instances of equity from micro-aggressions to criminal actions.

Please note that we at the Stephen Stewart are duty bound as mandatory reporters to report any known or suspected instances of child abuse or neglect. However, for all instances we are committed to maintaining privacy and safety of our students and will do so to the extent the law allows. Reports will remain confidential and names will not be mentioned when addressing and investigating, to the best of our abilities.

All reports will be investigated, and ALL attendees are subject to scrutiny.
