Thank you for your interest in Huntington Ballet Academy classes for Fall 2023. Please use this form to formally register for the following programs. In addition to the registration form, a Student/Parent Contract & Invoice will be sent with the total cost. Payment is due in full plus a registration fee ($60) however we offer payment plans for our students that allows them to pay over time. The details are set forth below. Any questions? Payment details:
- Tuition is non-refundable. Once registered this is an agreement that the full amount of the tuition for the year must be paid whether the lessons/classes are completed by a student or not.
- Payment is due in full with registration
- (3) payment plan: First payment due September.

- Monthly payment plan for 32 -week programs: credit card on file - a 3% fee will be added to the invoice
- A (10% discount will be applied for multiple programs and/or siblings - no discount shall exceed 15%.
- There is no cost to use Venmo/Paypal if payment is not labeled for goods and services.
