• I understand that if accepted for a Music in Hospitals & Care Cardiff Half Marathon place, I am undertaking to raise as much sponsorship as possible with a minimum of £250 (80% to be raised by 4 September 2022, 100% to be raised by 6 November 2022).

• I confirm that I will be 18 years of age by the date of the Cardiff Half Marathon and that, to the best of my knowledge, my general state of health and fitness is good and I take full responsibility for my training. I take part at my own risk.

• I give Music in Hospitals & Care permission to share my details with the event organiser, Run4Wales.

• I give permission for Music in Hospitals & Care to use photographs or videos taken of me to promote the charity and event on its social media.
