**THIS FORM IS TO BE COMPLETED BY A PARENT/GUARDIAN ON BEHALF OF THEIR CHILD(REN).  PLEASE USE THE FORM ONCE PER FAMILY.  INCLUDE ALL STUDENTS AND RESIDENTS UNDER 18 INCLUDING GRADES PRE-K-12 AND RESIDENTS WHO ATTEND OTHER SCHOOLS (PRIVATE, CHARTER, VO-TECH, CHOICE, ETC)**

Mount Olive Township School District is providing food for students/families prior to the Holiday break on Monday, December 21st.  We are requesting that orders be placed in advance to ensure we have enough for everyone interested.

ALL FOOD WILL BE PROVIDED AT NO CHARGE.

Each child will receive the following:
- Dry Goods - Breakfast items, Box of Dry Pasta, Graham Crackers and Raisins
- Frozen Goods - Muffins, Pizzas, Chicken Patties, Hamburgers, Fruit Cups, and Frozen Vegetables
- Refrigerated Goods - Mozzarella String Cheese, Marinara Sauce, Lettuce, Apples and Oranges
- Hamburger buns
- Juice - 48 oz. Bottle
- Milk - Gallon
- Allergy-free treats
- Several FUN activities!

DEADLINE IS WEDNESDAY, DECEMBER 16th at 5 PM to place your order.  Orders submitted after Wednesday at 5 pm will be accommodated based on product availability.

Each series of bags will provide 14 full breakfasts and 14 full lunches for each student.  We will provide a suggested daily menu and any cooking instructions needed.  However, each student/family can consume the food provided as they prefer.  All food will be packaged to improve safety for all our customers.  Menu and list of items are subject to change.
