In United STL Sports complex during the dates set forth in this application, and in consideration of your acceptance of the application, I / we hereby release United STL Academy, Roma Land LLC, and United STL Training center and the locations where United STL Academy training sessions will be conducted and all their respective staff, from all claims on account of any injuries which may be sustained by my / our child while attending United STL Academy activities. I / We further covenant not to sue United STL Academy INC, Roma Land LLC or United STL Training center for any injuries sustained and indemnify and hold United STL Academy, Roma Land and United STL Training center blameless for any claims which may hereafter be brought by my / our child as a result of any such injuries or death.
