Photograph/Video Agreement: The parent/guardian gives permission for Oak Bend Church to use child's photograph or video tape publicly.  Images may be used in print publications, website, and social media.  No child will be identified by name. *

Participant's Agreement: The parent/guardian acknowledges and accepts the risks of physical injury associated with participation in the Oak Bend Church Vacation Bible School 2025.  Except for gross negligence on the part of the sponsor, the parent/guardian accepts personal financial responsibility for bodily or personal injury or property damage sustained during Vacation Bible School 2025.  Further, the parent/guardian promises to hold harmless Oak Bend Church and its representatives for any injury to the participant related to Vacation Bible School 2025, or for any property damage.  Further, the parent/guardian hereby authorizes the representatives of Oak Bend Church to seek and provide medical treatment as deemed necessary and appropriate by such representatives in their sole and absolute discretion.  If any dispute over this agreement or claim for damages arises, the parent/guardian agrees to resolve the matter through a mutually acceptable arbitration process. *
