Please fill out this form in it's entirety to register your child(ren) for Faith Formation at Annunciation Church.
Please know that if your child is preparing to receive the Sacrament of First Communion (no earlier than 2nd grade) or the Sacrament of Confirmation (no earlier than 10th grade) that both of these Sacraments require no less than two years of preparation prior to receiving the Sacrament.  
The cost of the program this year remains $25.00/child and any child receiving the Sacrament of First Communion or Confirmation is asked to pay a $40.00 Sacramental fee.  If any of these fees cause a financial hardship please indicate that on this form in the required spot and someone will be in contact with you confidentially to discuss this.  We never turn anyone away for an inability to pay.
