LIABILITY WAIVER
I am aware that participation in any Competitive Sports Academy events, camps, programs, trainings and team games has some inherent risks and injury can occur. On rare occasions there injuries can be serious. In consideration of my child being allowed to participate in Competitive Sports Academy events, camps, programs, trainings and team games.

I, the parent/guardian, assume the risk of all injury and agree not to sue Competitive Sports Academy, the directors, coaches, assistant coaches, volunteers or any staff members for any and all injuries caused by or resulting from participating in any Competitive Sports Academy events, camps, programs, trainings and team games.

MEDIA RELEASE
By signing the consent form below, I give permission to:
• Use photos and/or videos of my child in CSA’s social media accounts (Website, Facebook, Instagram, Twitter, TikTok and YouTube), and for marketing and promotional purposes. (Please note that your child's name may appear in the photo/caption)
• Have my child recorded or photographed by a staff member or volunteer.

In providing this consent, I acknowledge that any photos/videos posted on CSA’s social media accounts and website may be widely circulated to the public, and may be viewed by anyone with access to the Internet. If I provide consent, and then later revoke my consent, I acknowledge that although the photos/videos will be removed from our social media accounts, it may not be possible to remove all traces of personal information from the Internet. All such images and recordings are property of Competitive Sports Academy.

AUTHORIZATION AND RELEASE
I hereby consent to the initiatives described in the letter above and I consent to this authorization and release.
