PLEASE NOTE:  THIS FORM IS DOES NOT CONSTITUTE REGISTRATION FOR A WINTER ATHLETIC  TEAM.  IT IS MERELY FOR SURVEY PURPOSES ONLY.

This survey will allow the Winter Coaches to create a database of information as we approach the season.  You may fill out the sign-up sheet more than once if you are deciding between sports, but will only be allowed to participate in one sport per season, as per NJSIAA regulations.
