Ottawa Crescent: Technology Request and Agreement 
Please complete the form below if you require a school Chromebook - or your child's SEA Chromebook - sent home to access remote learning (one form per family). We will try our best if you request multiple devices, but depending on Chromebook numbers and demand, we may only be able to loan one school device per family.  

Please make all requests using this form, before Monday, November 7th at 3pm.  Devices will be sent home with students on Tuesday, November 8th.  Alternatively, requested devices can be picked up from the school on Wednesday between 8:30am & 3:00pm.

Do you agree to the following technology at home agreement?*

1. Only Board approved software is to be loaded onto the computer by Board personnel. This ensures the proper configuration of the system. 

2. The board's Acceptable Use of Digital Technology  (Policy #318) will apply at all times and locations. 

3. Repairs may only be done by an Upper Grand District School Board repair technician. 

4. Equipment will be used with the provided electrical supply cords only. 

5. Equipment will be plugged directly into a wall receptacle or CSA approved power bar. 

6. Technology support including repairs and technical advice is not available in the home setting or in other locations outside of the school building. 

7. Equipment is to be handled with care to avoid damage. I understand and agree to follow the above conditions regarding the use of Upper Grand District School Board technology at home. 

8. This equipment will be returned to the UGDSB immediately upon request. 

While certain web browsing safety technology has been installed on this chromebook, I acknowledge that no software can provide 100% protection from inappropriate websites. I also acknowledge that it is my responsibilty to use the equipment for education related services as directed by staff in the UGDSB.
