If no form is completed by March 16th, After-School staff will place student in crews as they see fit. If a student's After-School schedule changes after March 16th, options will be significantly limited. If a student behavior disrupts their crew, they will be required to move to another crew as assigned by After-School staff.

Dear Parents/Guardians

Please complete this online and keep the paper copy at home. We're trying something new! Once homework hour is complete, we would like to allow your students a choice (with your help) about which activities they'd like to participate in for the enrichment time. Students can choose from any of the following options; however, tips for best results are listed. Selections made will stand for the 4th quarter. Your signature at the bottom of the form is required. Some groups have a limit and all selections will be first come, first served. We will be looking forward to your valuable feedback.

Mondays *

PowerUp P.E.! Exciting exercises and group games. (All ages welcome.)

Spring Spruce Up! Students will assist staff and community in cleaning and organizing projects. (All ages welcome - possible walking field trips.)

Service-Learning at Cornell Health Services! Games, read-aloud and friendships to last a lifetime! (2nd grade and older. - Walking 5 blocks there and back right after school: Due to dinner service at the Care Center, this group starts homework after snack at 4:30.)

Active Fun! Dodgeball, Rockwall, Big Base and more! (4th grade and older.)

Extra homework help please. (This is in addition to the already scheduled homework hour: all ages welcome.)

My child does not attend After-School enrichment on Mondays.

Tuesdays *

Lego Table! Students will build Lego creations with kits, themes or freestyle. (All ages welcome.)

Outer Space Science! Creative hands-on activities including slime! (2nd grade and younger.)

Intro to Agriculture! Plotting & 3D models included in this hands-on learning. (2nd Grade and older.)

After-School Newsletter! Jr. Journalists will interview, photograph, edit, publish and present! (3rd grade and older.)

Movie Mania! Look at movie themes while problem solving with the characters. (4th Grade and older.)

My child does not attend After-School enrichment on Tuesdays.

Wednesdays *

PowerUp P.E.! Exciting exercises and group games. (All ages welcome.)

Scratch, Gimkit & Kahoot in the Computer Lab! (All ages welcome.)

Outdoor Learning! Geocaching, compass training and more. (3rd grade and older: many walking field trips; plan to stay until at least 5:45.)

Service Learning! Clean, plant & paint at local food pantry. (This session is open only to 5th grade or older students. Walking 6 blocks there & back each week.)

Extra homework help please. (This is in addition to the already scheduled homework hour; all ages welcome.)

My child does not attend After-School enrichment on Wednesdays.

Thursdays *

Talent Tune Up! Students who hope to participate in the talent show May 21st will get expert tips and time to hone their skills. (All ages welcome.)

Recycle! Creative hands-on activities including a bird feeder, pencil holder & kaleidoscope! (2nd grade and younger - Walking field trip May 14th.)

Calm and relaxation. Includes yoga and making DIY stress balls. (2nd grade and older.)

Breakfast Bunch! Practice practical kitchen math and follow written directions while making favorite breakfast items. (3rd grade and older. Max 10 students.)

Community Sightseeing! Enjoy the outdoors of Cornell. (3rd grade and older, plan to stay until 6:15, field trips likely.)

My child does not attend After-School enrichment on Thursdays.
