JH Fall Sports Sign-ups 2024-2025
Current 6th and 7th graders: Please complete the following information if you are interested in participating in a fall sport for the 2024-25 school year.  You will need to submit a new sports physical packet for the school year that is dated on or after May 1, 2024.

Please mail or submit physical forms to the Middle school main office. Office hours are Monday – Thursday, 7 am-4 pm. Do not submit paperwork to coaches.
