Fun Soccer Academy - May & June Registration 2023
The Fun Soccer Academy coaching staff will stive to provide a safe and fun soccer learning experience where we will focus on games with finishing on goal.  

When?  
-  May:  12th Friday ~ Indoor,  19th Friday~ Indoor, 26th Friday~ Outdoor TBA, 31st Wednesday ~ Indoor & June 16 ~ Indoor. 
- 6pm - 7pm

Where?
- Stryker Sports Complex 2999 N Greenwich  Wichita, KS 67226

Who?
-  Boys & Girls of birth years between 2011 - 2018.

Cost?
  $99 per camper.

How?
1. Complete this form by Friday, May 5th.  

2. Submit payment upon registration in one of the following ways: Venmo - @dylangruntzel, Apple Pay - (316) 518-4846. Contact Dylan Gruntzel if needing another form of payment such as mailing a check.

3. No refunds after the deadline of registration.
 
4. Due to limited spots, confirmation of acceptance will be emailed within 48 hours after the deadline of registration.

5. If we are unable to accommodate you, you will be notified and the payment will be returned and checks will be shredded.

I understand that, as a condition of admittance as a participant, the undersigned, on behalf of all parents and guardians, and behalf of the applicant, hereby releases Fun Soccer Fridays Academy all of the coaches and employees or agents form any and all liability from injury or illness, mental or physical suffered by the participant during or related to this training. *
