This form should be completed separately for each school or program where the transcripts are to be sent. (i.e. 3 scholarships= form should be completed 3 times)

SCHS cannot guarantee on-time delivery for requests submitted with fewer than fifteen school days notice.

Please confirm you have completed the “Consent for Release of Student Records in Support of Post-secondary Applications” form, and e-mailed it as an attachment to the transcript office at SCHStranscripts@fcps.edu. (This waiver will only need to be completed ONCE, with your first transcript request.)

Transcripts will be sent with a School Profile. Any recommendations to support scholarship applications must be requested from the counselor directly and are not automatically included by submitting through this form.

Please Note: South County HS does not send official standardized test scores (SAT, ACT) to colleges. You must request them online via College Board (www.collegeboard.org) or ACT (www.actstudent.org).
