PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS. BY SIGNING THIS WAIVER AND RELEASE OF LIABILITY ("Agreement"), YOU ARE AGREEING TO RELEASE AND WAIVE CERTAIN LEGAL RIGHTS.

This waiver must be signed by each player independently if they are part of a league or a club. Each person aiming to play at La Liga must have filled this waiver and show it at the main office on the day they come to the facility.

PARTIES:

This Waiver and Release of Liability is entered into between La Liga Indoor Soccer Inc. (hereinafter referred to as "La Liga"), located in Toronto, and the undersigned participant or participant's legal guardian, leagues or clubs (hereinafter referred to as "Participant" or "Guardian"), collectively referred to as the "Parties." 

ACTIVITY DESCRIPTION:

Participant acknowledges that La Liga is a sports complex located in Toronto that rents 3 hardwood floor courts for basketball, indoor soccer, pickleball and other sports. Participant or Participant's legal guardian desires to use the facilities and engage in activities conducted at La Liga, including but not limited to playing basketball, indoor soccer, pickleball, or other sports (hereinafter referred to as the "Activities").

ACKNOWLEDGEMENT OF RISKS:

1. Participant acknowledges that engaging in the Activities involves inherent risks, including but not limited to:
2. Physical exertion, strenuous physical activity, and the potential for bodily injury, including sprains, strains, fractures, and other injuries.
3. Collisions, falls, and contact with other participants, equipment, or surfaces.
4. Tripping hazards, uneven playing surfaces, or other conditions that may cause accidents or injuries.
5. The negligence or misconduct of other participants, agents, or contractors.
6. Participant understands that the risks associated with the Activities cannot be eliminated completely, and despite the implementation of safety measures, accidents and injuries may occur.

WAIVER AND RELEASE OF LIABILITY:

In consideration of being allowed to participate in the Activities, Participant, on behalf of themselves or the minor for whom they are the legal guardian, hereby agrees to the following:

1. Participant assumes all risks associated with the Activities, including those mentioned above, and agrees to release, indemnify, and hold harmless La Liga, its officers, directors, employees, agents, and contractors from any and all claims, demands, causes of action, liabilities, losses, damages, costs, or expenses, whether known or unknown, arising out of or in connection with the Activities or Participant's presence at La Liga's facilities.
2. Participant, or Participant's legal guardian, grants permission to La Liga to obtain or provide emergency medical care or treatment in the event of an injury or illness sustained during the Activities. Participant agrees to assume all costs associated with any such medical care or treatment.
3. Participant represents that they are physically fit to participate in the Activities and have not been advised otherwise by a qualified medical professional.
4. Participant agrees to abide by all rules and regulations set forth by La Liga regarding the use of its facilities, including any instructions provided by La Liga's staff or representatives.
5. Participant acknowledges and agrees that this Agreement shall be binding upon their heirs, executors, administrators, and assigns.
6. Participant authorizes La Liga to collect and use their personal information for the purpose of receiving communications from La Liga. We do not sell or distribute your personal information to any third party.

 ACCIDENT INSURANCE:

Executing this agreement will not preclude you from accident insurance coverage, subject to the terms and conditions of La Liga’s Insurance Policy.

 By signing below, Participant acknowledges that they have read this entire Waiver and Release of Liability, fully understand its contents, and voluntarily agree to its terms.
