I (the above-stated name address and telephone number and date), am the Parent/Legal Guardian of the above stated Minor/Rider.

I, and the Minor/Rider, are aware that the sport of motorcycling and racing involves many inherent risks, dangers and hazards, including but not limited to: terrain, weather, conditions of equipment, other riders and the vehicular traffic. I, and the Minor/Rider acknowledge these risks and consent to the participation and involvement of the Minor/Rider in the Victoria Motorcycle Club (VMC) events and activities.

I acknowledge that as the miners parent or legal guardian, it is my duty and responsibility to supervise and control him/her and I shall exercise the supervision and control during any and all Victoria Motorcycle Club (VMC) events and activities.

I and the Minor/Rider/writer, have read and understand the rules of the Victoria Motorcycle Club (VMC) and I have explained these rules to the Minor/Rider.

I (the above-stated name address and telephone number and date), release the Victoria Motorcycle Club (VMC) its committees, members, officers, employs, agents, volunteers, officials, and independent contractors and sponsors from any claim whatsoever for the loss and damage connected with any property lost her personal injury, including death, that I sustained while participating or preparing for any, or attending any Victoria Motorcycle Club (VMC) activity or event, whether or not such loss or injury case calls solely or partially by the negligence of the Victoria Motorcycle Club (VMC), its officers, employees, agents, volunteers, rider officials, independent contractors or sponsors.

I agree to hold harmless and indemnify the Victoria Motorcycle Club (VMC), its committees, members, officers, employees, agents, volunteers, officials, independent contractors and sponsors from any and all liability and property damage, personal injury or death suffered by the Minor/Rider or by third party as a result of my participation or involvement in any of the Victoria Motorcycle Club (VMC) activities.

I have read and understand the rules of the Victoria Motorcycle Club (VMC) and agree to abide by these rules.

I further agree that this release and indemnity agreement shall be effective and binding on my heirs, next of kin, executors, administrators, and assigns in the event of my death.

I certify that I am in good health and do not suffer from a heart condition or any other medical condition which in any way would expose him/her to an increased risk of injury and taking part or participating in any Victoria Motorcycle Club (VMC) event or activity.

I have read and understood this release and indemnity agreement prior to signing it and I am aware that by signing this agreement, I am affecting the legal rights and liabilities of myself, my heirs, next of kin, executors and administrators and assigns in relation to the Victoria Motorcycle Club (VMC) its committees, officers, employees, agents, volunteers, officials and independent contractors and sponsors.

By placing your name on this legal document You waive certain legal rights, including  the right to sue. Please read carefully before signing. 

By signing this document you agree that this indemnity agreement is active and valid for all VMC activities, event and use of VMC property you and the minor you are guaranteeing participate in or on while you are a paid up member of VMC.
