2019 Online Summer Reading Log for Memorial Middle School

Please complete this form to be entered in the Summer Reading Program. You must have read three books to submit your first form. You can submit additional forms throughout the summer. All submissions must be entered between the dates of Friday, June 14th and Friday, August 23rd.

Students may be asked by their ELA teacher or the school librarian to share thoughts on a selected book at any time.

There will be one grand prize winner of a $100 Amazon gift card per school. Also, two $25 Barnes & Noble gift cards per grade level will be awarded. All winners will be randomly selected through a raffle and will be announced in September. Happy reading!
