Thank you for your interest in playing 2019 ACF Fall! ACF Fall will be held on Saturday, November 2nd, 2019*. Please fill this form out once for each team.

*The Southeast site, at Georgia Tech, will be held on Sunday, November 3rd.

Discounts
Are you required to write a packet? *
Any team with at least two people on it who played a regular, collegiate, academic quizbowl tournament (either as a college student or as a high school student) prior to September 1, 2018 must submit a packet. If you are required to write a packet, or would like to write one for a discount, please contact fall@acf-quizbowl.com. Submitting this form is not sufficient to receive a distribution to write for Fall.
How many buzzers will you bring? *
To receive a $10 discount, each buzzer system must have at least 4 working units per side. If your school is bringing multiple teams, please put all of your buzzers on your A team's registration.
How many staffers will you bring? *
Is your school new to quizbowl? *
The "new to quizbowl" discount is intended for schools that did not send a team to any regular collegiate academic tournament (exclusively Novice tournaments do not count) since September 2018, and have no one on the team(s) claiming this discount who played those tournaments for another school.
Do you anticipate that this team will be shorthanded? *
A shorthanded team is comprised of 1-2 players.
Will your team be traveling more than 200 miles one-way? *
As determined by Google Maps.

Does your entire team meet ACF's definition of Division II? *
A “Division II” player is any player who first enrolled in a collegiate program in 2019-2020 or 2018-2019 (i.e. is any freshman/first-year or sophomore/second-year).
Does your entire team meet ACF's definition of Undergraduate? *
An “undergraduate” is a player who did not receive a BA or equivalent or higher degree prior to 2019-2020, AND who first enrolled in a collegiate program in a competition year in 2015-2016 or more recently.
Tentative Roster *
Please indicate your team's tentative roster. ACF understands that players may add or drop from teams, or move around between teams for the same school. You may indicate more than four players here. After each player's name , please indicate "(DII)" if they meet the definitions of Division II and Undergraduate, "(UG)" if they  meet the definition of Undergraduate but not Division II, and nothing if they meet neither definition, e.g. "Juan Perez (DII), Jane Doe (UG), John Smith." High school teams need not indicate Division II and Undergraduate eligibility.
Photo, Visual, and Audio Release
ACF may like to use media taken at 2019 ACF Fall.

I, the undersigned, consent to the use by the Academic Competition Federation of my image, voice, or both in photographs, videos, or audio recordings taken at 2019 ACF Fall, or any photograph, video, or audio recording reproduced either in whole or in part from photographs, videos, or audio recordings taken at 2019 ACF Fall. I understand that I am giving my consent regardless of whether these materials are used on social media, the Academic Competition Federation's website, or for advertising, publicity, or fundraising.

I warrant that I have the full right and authority to grant this consent.

In addition, I waive all claims to royalties, compensation, or damages based on the use of my image or voice, or both, by the Academic Competition Federation. I also waive any right to inspect or approve the edited photographs, videos, or audio recordings before their publishing.

I understand that this consent is perpetual, that I may not revoke it.

I warrant that I am at least 18 years of age and that I am competent in my own name insofar as this consent is concerned. I further attest that I have read this consent form and fully understand its contents.
