Raiders TKD - Travel Declaration Form
Dear Coaches, Trainees, Parents and/or Guardians,
 
To further enhance protection of our Trainees and Coaches against the 2019 Novel Coronavirus infection, Raiders TKD will be taking additional precautionary measures for the well-being of our members.
 
[1] All parents, trainees and Coaches are required to fill up the travel declaration form before Taekwondo training class resume.
 
[2] We will also be requesting for parents/guardians not to enter the training ground and to wait for your child/ward at the entrance area.
 
[3] All trainees are also expected to bring their own thermometer as we will begin our lesson with temperature checking. Trainees will triggered temperature of 38 degrees and above, will be asked to leave the class.
 
[4] If you/your child is unwell,  have any symptoms such as cough, fever, or flu, please get medical attention immediately and stay at home to rest.
 
We seek the kind understanding of all to adhere to the precautionary measures put in place and be socially responsible during this period.
