Duke Pizza Lunch Order Form '22/23
Please use 1 form per student.  Ordering pizza lunch is a 2 step process: 1) order via this form and 2) make payment for your order - etransfer is the easiest but by cash or cheque is also an option (payment to the office by the deadline, envelope clearly marked "Pizza Lunch".  Cheque payable to Duke of Connaught SC.)  You can make 1 payment for multiple students (just need a separate form for each student).

Make sure you select the correct teacher for each student - parent volunteers who run pizza lunch do not have access to class lists.
