Hornets Registration Form
I am the parent/guardian for the child/children listed below whom accepts responsibility for registering and committing to the Jr. Hornets football and cheerleading programs. Only I and the authorized person I note below may address any and all concerns in regards to said child/children listed on this contract. My authorization of the person I have noted as well as I agree and have notated the following list of requirements by the Jr. Hornets.
Participation: The Jr. Hornets will develop my child through techniques and drills, understanding of the game and fundamentals from attendance & participation at practice, and I understand game time must be earned and is limited based on experience & skill level
Competition Level: The Jr. Hornets participate in a competitive league both in cheer & football in which teams are vying for District Titles, Playoffs and State Championships. I accept the coach’s responsibility to position my child according to skill level in which he/she feels is best for the team’s overall success.
Playing Time: TYFA does not have a mandatory play rule and I understand that the Jr Hornets does not have a mandatory play rule as well. I understand that the Jr. Hornets view playing time as earned. TYFA is a competitive league and safety is priority if a child is not ready to play.
End of Season Rewards: Understand that we forfeit all end of season rewards if my/our child does not complete the entire season to include playoffs & state games or my/our volunteer hours are not fulfilled. If we owe any money and fail to complete mandatory fundraiser, fail to turn in equipment, or return damaged equipment outside normal wear and tear, I understand that I am in jeopardy of forfeiting end of season rewards.
Volunteer Hours: Through out the season we will need volunteers to help with home games.
Practice Field: I understand that I am not allowed to come on the practice field unless there is an emergency.
Game Field: I understand that during the game I will not come onto the game field during halftime or during the game unless there is an emergency. End game parent celebration tunnel is ok.
Fundraiser: I/We are responsible for the entire dollar value of the fundraiser regardless of ability to sell.
Certification: I/We are responsible for ensuring our child/children attend mandatory certification and if my/our child/children do not certify will not be allowed to participate for the season and no refunds will be given.
Distribution of Equipment or Uniform: My/our child’s/children’s equipment or uniform may be retained by the Jr. Hornets until payment or fundraiser has been paid in full by set dates.
Monetary Donations: Any donations requested by team managers or coaches are NOT required and that the only monetary responsibility I/we are responsible for is registration costs and mandatory fundraiser.
24 hour rule: After a win or loss, please address any concerns with your coach 24 hours after competition.
