Town of Madison Recreation Department  Participant Emergency Card, Liability Release, and Photograph Release Form
Thank you for participating in Town of Madison Recreation sports, programs, and activities. In order to continue our programs, we need to incur a small fee of $35 for resident players and $45 for non-resident players. The fee is designed to cover expenses associated with uniforms, equipment and program/league fees. We recognize that these are financially challenging times for families, but small fees help build a self-sustaining Rec. program in Madison. This will help make our program less dependent on our town budget and donations and helps make each program supported by its participants. 

Please complete and sign this form as follows:
  • Enter the required participant and parent/guardian information.
  • Enter the required information and sign the Emergency Card.
  • Fully read and sign the Liability Release and Photograph Release. 
  • Submit the form once completed.
Remember to click Submit, and complete a separate form for each participant.

Practices will be on Tuesdays & Thursdays at 3:30/3:45-5pm, starting in April depending on the field conditions after the winter.
