Helpful Information Before You Begin
Please fill out the following requested information correctly. By doing so, it will ensure your registration and payment without any delays.

**NOTE** This form should be used for IN-PERSON VENDORS ONLY.

Other helpful tips include:
1. Keep track of costs as you complete the form. There will not be a tally of total cost when asked.
2. Please have your accounting department contact information ready.
3. You will receive a confirmation email to the email provided after the form is complete (may take up to 24 hours)
4. Pay close attention to the final section regarding payments & refunds
Keeping You Safe During the Conference
As partners with Little America and to ensure a safe welcome to our membership and vendors TAA will set forth the following expectations in conjunction with those of our host Little America Hotel, and Coconino County Health and Human Services.

- All attendees must wear face coverings for all in person conference events
- All attendees must practice social distancing in break out sessions, general sessions, and during meals and group events.
- All attendees must cease participation if they begin to display symptoms of COVID 19 during the conference.
- Any registered member must cancel their in-person attendance if they are exposed to symptoms or begin to display symptoms within 14 days of the TAA event.
