The purpose of this follow-up survey is to prepare our buildings to best meet the needs of individual students and families. We share information here about the delivery models BCSD has developed at this time so that you can determine your student's needs. As such, this survey will not be anonymous. Also, because we recognize that needs for students can be individualized even within a family, we ask that you complete one survey for each child living in your household who attends BCSD. The survey will take no more than 5-10 minutes to complete.

Who will see my responses? Your responses will be seen by district and building administrative staff. As supports are needed, data will be shared at the building level with other staff who will need this information to provide needed services and resources. Summaries of the data may also be shared with the Boone Board of Education. (names will not be shared)
