Completion of this form assumes legal and financial responsibility for all policies and fees outlined in this document, including but not limited to acknowledgement of processing fees and the withdrawal policy. Enrollment is a two-step process.

Step 1 - Form: Please complete this form (one per student) to enroll for a class. Please follow instructions at the end and click SUBMIT to ensure that your form is complete. An email confirming that you are registered will be sent to the email address listed below.

Step 2 - Please watch your email in the coming weeks for an invoice to complete payment. CTC does not collect payment with this form.
