Board and Commission Application Form
The City of Pleasant Ridge thanks you for your interest in serving as a citizen representative on a City board or commission. Your candidate questionnaire will be kept on file for a period of two years and entered for consideration for posted openings on any Commission for which you have expressed an interest. Please feel free to submit an updated candidate questionnaire at any time.

Please be advised that the information contained in this questionnaire is not confidential and will be reviewed by the Mayor and City Commission. Out of respect for your privacy, the information in the first section, including your address, phone number, email address, education, and occupation will not be made public as part of any City Commission agenda packet or otherwise published or distributed, but that information is subject to Freedom of Information Act (FOIA) requests.

Your responses to the questions in the second section about your interest, experience, and qualifications will be included in City Commission agenda packets which are published and made available for public inspection on the internet, so please include only information that you are comfortable being on the public record.
