On behalf of my minor child I hereby give permission for my child to participate in the Strategy Gaming Club at the Cerberus Den located at, 20 N. Grand Ave. Suite 100, Fort Thomas, KY 41075. I hereby warrant that both myself and my child are familiar with the risks associated with participation in Cerberus Den activities. I acknowledge that my child’s participation in this program is wholly voluntary and financial and legal responsibility of my child’s actions and/or behavior is on myself.  

I do hereby agree and consent to my child’s participation in the Strategy Gaming Club from the scheduled hours posted on the club site and do assume all risks and hazards which are conducted as part of the associated activities. I understand that no child under the age of 10 will be left unattended by parent(s) or legal guardian(s) in the Cerberus Den, and therefore confirm my child is at least 10 years old or within 3 months of reaching 10. I understand that my child must be picked up promptly when the event ends (latest being 1 hour before closing) and must have all forms completed including but not limited to; Liability Release, General Contact and Emergency Contact Form, Program Drop Off/Pick Up Agreement, Photo Release Agreement. I hereby release, absolve, indemnify, and hold blameless Cerberus Den, its officers, directors, employees, contracted employees, independent contractors, instructors, agents, organizers, and volunteers of any and all liability for damage, injury, or expense of any kind arising out of or connected with my child’s participation in Strategy Gaming Club at Cerberus Den. I understand that in case of a medical  emergency, my own personal medical plan will be used if available; if not, medical aid may be sought while I am being contacted. In the event of any illness or injury, I hereby consent to whatever examination, diagnosis, or treatment and the hospital care from a licensed dentist, physician, and/or surgeon as deemed necessary for my child’s safety and welfare. I understand that the resulting expenses will be my responsibility and not the Cerberus Den, its employees, or any affiliates. As a condition of participation in the Cerberus Den Strategy Gaming Club by the child named in this application, I acknowledge that I have read this consent form, and knowingly, on behalf of my child, assume all of the risks associated with participating in any way in the Cerberus Den Strategy Gaming Club.

As consideration for being permitted to participate in the Strategy Gaming Club, I hereby agree that I, my assignees, heirs, distributes, guardians, and legal representatives will not make claim against, sue or attach the property of the Cerberus Den, or their Affiliates or the supplier of any of the equipment used in these activities for injury or damage resulting from negligence, failure of care, omission or other acts, however caused by an employee, agent or contractor of Cerberus Den and any affiliate as result of incidental to my child's participation in these activities.  I hereby release, to the fullest extent permitted by law, the Cerberus Den and the affiliates, from any and all actions, claims, demands, or liabilities that I, my assignees, heirs, distributes, guardians, and legal representatives now have or may hereafter have for injury or damage resulting from my child’s participation in these activities irrespective of whether the same is based on breach of warranty, negligence, strict liability or any other theory or recovery.  Parent/Legal Guardian acknowledges that Cerberus Den Strategy Gaming Club is an out-of-school program and not a licensed childcare or daycare facility.  I have read this agreement and agree to the conditions stated above. 

In addition to being permitted to participate in the Strategy Gaming Club, it is understood that a form of payment will be kept on file at the Cerberus Den.  It is understood that I give permission for the card on file to be used when my child(ren) decides to damage, open, or any other act that would indicate an understanding of financial responsibility by me, the parent.  I understand that Cerberus Den is not responsible for damaged items or theft of personal items.  It is the responsibility of myself and my children and all accountability lies on the aforementioned.  It is understood that my child(ren) may engage in trading or exchanging of Trading Cards (TCGs) or other collectibles in exchange for money or other items of understood and agreed upon monetary value at the time of trade.  I understand all trades are final and deemed fair at the time of agreement as it pertains to all parties involved including the Cerberus Den, volunteers, affiliates, and patrons of Cerberus Den. I grant permission that pictures, videos, social network posts and accounts of the event(s) my child(ren) participate in to be utilized by Cerberus Den for promotion and business use. I acknowledge that I have read this consent form, and knowingly, on behalf of my child, assume all of the risks associated with participating in any way in the Cerberus Den Strategy Gaming Club.

I understand the membership fee of $25 is due at time of signup, and participation in the Cerberus Den Strategy Gaming Club is only permitted with the fee paid in full.  Additionally, it is understood that the fee allows for membership for one year for one child, and the renewal fee will be processed on the annual anniversary date. Participation in the Cerberus Den Strategy Gaming Club is subject to termination in the case of annual fee lapse due to nonpayment of the $25 membership fee.  Lapse of membership eliminates any and all perks provided by being a member of the Cerberus Den Strategy Game Club including, but limited to discounts, food programs, membership programs, etc.  Membership may be reestablished dependent on the annual membership fee being paid in full.  The renewal date of lapsed memberships occurs on the date the membership fee is paid in full.
