I hereby give permission for myself (or my child) to be interviewed and/or images and performances to be captured in a photograph or video that may be used by On the Off Beat Music School, Inc. for the purposes of media coverage, website promotion on www.ontheoffbeat.ca, social media promotion on On the Off Beat Music School, Inc.'s social media accounts (e.g. Facebook, Twitter, Instagram, YouTube, Vimeo or other) and hereby release On the Off Beat Music School, Inc. from any and all claims whatsoever in connection with the use, reproduction, publication of the images thereof. Names will not be used.
