                                 Madison Elite Waiver

Madison Elite’s number one priority is the health and safety of our camp participants and staff. Our goal is to hold a safe event, recognizing it is not possible to remove all risks of injury or illness, in particular COVID-19. We will follow CDC and public health recommendations, federal, state, and local regulations applicable at the time of each camp.

I hereby authorize the personnel of Madison Elite Volleyball Club and the 2022 camps to act for me according to their best judgment in any emergency requiring medical attention.

I hereby waive and release Madison Elite Volleyball Club, its affiliates, all sponsoring, host facilities, employees and agents from any and all liability from any injuries incurred while at camp or for COVID 19 cases arising because of camp attendance.
