£10 per day, £30 for 3 days (Non-refundable)

Parent/guardian please fill one form per child. After you submit the form, we will contact you by email about the details of the bank transfer.

If your child falls sick during the time of Summer Holiday Club or if he/she has recently contacted with any contagious disease, please keep your child home.

If your child requires an inhaler or an allergy pen, we will be happy to assist he/she with your written permission, which will detail a full explanation of how to use the items involved. However, if your child requires regular medication to be administered during the club, we would ask you to come in and administer it.

We will take photos during Summer Holiday Club. The photos are only for our church record and event promotion. A copy of a group photo will also be given to your child. Would you like us taking photos for your child? *
