Medical Information - Please state any disability, medical condition or allergy, which we may need to know about for your child, or maybe relevant to their engagement in physical activity (e.g. epilepsy, nut allergy etc). If your child is on the ASD spectrum there are some reasonable adjustments we can make to help your child have a more enjoyable experience. Please talk to one of the teachers if you would like more information. This information will be treated as confidential and stored securely and will be accessible only by Echo Echo staff in accordance with GDPR Laws.

We will hold your details privately and securely for up to 5 years after which time your consent will be requested to continue holding them otherwise they will be destroyed.
