Student Government 1/4 Zip Jacket Order Form
Student Government has designed the following 1/4 Zip Dri-Fit Jacket which has been approved to be worn EVERY DAY over the regular uniform polo shirt. This is for Middle School students ONLY! Parents are welcome to order a jacket for themselves as well. The cost of the jacket is $35.  A small portion of that cost will go directly to the Student Government for future events.

Please complete the following form to order a 1/4 Zip Jacket for you and/or your child by December 16, 2019.  Please fill out the form completely and be sure to indicate the number and size you want by checking the appropriate box.  Charges for the jackets will be added to your Unity School FACTS account.  For any questions regarding the jackets, please email Mrs. Ferguson at bferguson@unityschool.com
