This form is used to fill in after driving but also to check before driving. It is a must that you fill in this form correctly or as you can read in the car rules you might suffer consequences. All answers given here will be deleted on the 05/01 every year.

Make sure to use the same email as the one you used to register! It is most likely your Chalmers mail (CID@chalmers.se).
If you are driving for a Committee, use COMMITTEE@dtek.se.

Double check so that the following is correct, if not send an email to dbus@dtek.se explaining what is wrong. It's recommended to send an image as proof. If we don't recieve an email we will charge you accordingly to the information given here.
