Please fill out the form below to enroll in a culinary adventure at La Toque de Cindy and let the fun begin. To see full descriptions read here www.cindytoquecooking.com. An invoice will be sent after your registration is submitted. Note we may be wearing masks based on the requirements of the County of Santa Clara. Your registration fees (minus transaction costs) will be fully refunded if for some reason your class needs to be cancelled. Register for afternoon classes must be at least 24 hours in advance.

Do you agree to the cancellation policy *

I agree to the cancellation policy: Your space is reserved when payment is received. Request a refund 14 days in advance (by April 1 for summer camp) to receive the total fee minus a $25 processing fee per student. For requests 7-13 days in advance (or after April 1st and at least two weeks prior to the first day of camp) to receive a 75% refund of the total fee paid per student cancellation. There is a $25 fee for switching camp sessions. Sorry, no refunds within two weeks of the start of camp or 7 days before a private event, tour or class. Classes with 1 student subject to change

I agree to the waiver. The undersigned parent or guardian assumes all risks in connection with the family’s participation in this activity. I, the undersigned participant/guardian, intending to be legally bound, do hereby for myself and heirs, executors, administrators and assigns, forever waive release and discharge Cindy Roberts and La Toque de Cindy, all La Toque de Cindy employees and contractors and agents from all liability, claims or demands for any damage, loss or injury to the student, the student’s property, or parent’s property or to myself in connection with participation in these activities, unless caused by the negligence of Cindy Roberts and La Toque de Cindy. I do hereby certify that to the best of my (our) knowledge and belief said parties are in good health. In case of illness or accident, permission is granted for emergency treatment to be administered. It is further understood and agreed that the undersigned will assume full responsibility for any such action, including payment of costs. I attest and verify that my student(s) enrolled physically fit and able to participate in this event and acknowledge that I am aware of the inherent risks including Covid-19.

I agree to the food handling policy. Most food will be eaten in class. If food is taken home it must be refrigerated within 30 minutes. La Toque de Cindy will not be held liable for food related issues. Food handling during class will comply with the policies and procedures of the Health Department of the County of Santa Clara
