Recreational "Studio Dancer"/Pre-Competitive/Homeschool Program  Enrollment Form
Welcome to The Pointe PAC!!!  We are excited to begin our 20th year of dancing!  Our goal is to instill the art of dance while having fun and creating a loving and accepting environment for our students.  Your dancer will be learning life skills, musicality, flexibility, strength, and movement.  We pride ourselves on training the  most technical dancers along with building their confidence and giving them a skill set that will carry them throughout their lives.  We welcome you and are excited for you to join us this year!!!

"Studio Dancer Classes"
Monthly Tuition:
                              30 minutes per week - $30 per month
                              45 minute per week - $40 per month
                              1 hr per week - $50 per month
                              2 hrs per week - $95 per month
                              3 hrs per week - $125 per month

                              Pre-Competitive Program (3hrs per week, 2 local competitions) $150 per month (extra fees will apply)
                              (Please see the front desk for more info on Competition/Costume fees for Pre-Competitive Program)

                              Homeschool Dance Intensive Program (Tuesdays 9:30am-12:30pm)
                              Technique 1.5hr + Lesson/Journal Session - $75 per month
                              Ballet Technique 1.5hr + Lesson/Journal Session - $75 per month
                              Homeschool Package (both sessions listed above) - $125 per month

I understand that I am responsible for the total amount signed up for the Summer enrollment form for 2019.  I understand there are NO refunds.  I understand The Pointe PAC will invoice my account once this form is submitted.  The Pointe accepts payments via bank draft, credit card, check or cash.  

Monthly payments are due by the 10th of each month, paying after the 10th will result in a late fee.  There is $25 fee on all returned checks.

Submitting this form will prompt our office to send you an invoice for your enrollment fee.  This fee will secure your child(s) placement for classes.

What to purchase & what to wear:
(We recommend "Queen of Hearts" Dance Boutique in The City of Moore Shopping Center)

18month/2yr classes:  Leotard & Tights, hair up off the neck, ballet shoes

Creative Classes:  Any color leotard & convertible/footed tights, ballet slippers, tan mary-jane tap shoes, hair up off the
                                neck

Pom/Jazz/HipHop Classes:  Any type of Dance Wear, tan "NeoFlex" BLOCH Jazz Shoes, hair up off the neck

Tap/Jazz Classes:  Any type of Dance Wear, tan "NeoFlex" BLOCH Jazz Shoes, tan maryjane style tap shoes, hair up off the
                                  neck

Pre-Competitive Classes:  Leotard & Tights for ballet, hair in a bun, ballet slippers
                                               Any type of Dance Wear for Jazz & Tap, hair up off the neck, Tan "NeoFlex" BLOCH jazz shoes
                                               Tan "No Laces" Tap Shoes BLOCH

Tumbling Classes:  Any type of workout/dance wear, hair pulled up, we tumble with bare feet

Pom Team Prep Classes:  Any typ of workou/dance wear, hair pulled up, jazz shoes

Ballet Technique:  Black leotard & Pink tights, hair in a bun, split sole ballet slippers

Tap Technique: Any dance wear, hair up off the neck, tap shoes
