This form is for Team Nominations for the 2021 Season 1 BMTA Junior Girls Competition starting February 11th. Before completing this form please ensure you have read and understood the full Conditions of Entry document available at http://bmta.com.au/junior-team-nominations/

*If you have organised to play for a club team, e.g Southern Storm, UQ Rebels, All Hallows, Pink or STM, then there is no need to compete this form, your club will nominate your team for you. This form is only for independent teams who are not part of a Club.

Please note, this form does not have a payment attached however Team Nominations are not confirmed until the $150 team deposit is received by BMTA. Following the completion of this form, BMTA will send the nominated email address an invoice for the team fees.
