According to LA County Department of Public Health requirements, all Claremont AYSO players and volunteers who are age 12 years and older who have not provided proof of Covid-19 vaccination must be tested weekly.

These players and volunteers should submit this form on a weekly basis to inform the region of their testing results. Please respond separately for each person who is required to test.
